Patient Enrollment in Portage Medical Family Health Team

Portage Medical Family Health Team includes physicians, nurse practitioner, registered nurses and a range of other health care professionals who are committed to working together to provide comprehensive accessible, coordinated primary health care services to their enrolled patients.  Portage Medical Family Health Team emphasizes prevention and healthy living as much as treating illness.

Enrolling with your Family Physician who is part of the Portage Medical Family Health Team, ensures that you as a patient has access to primary health care treatments or advice, 24 hours a day, seven days a week, through a combination of regular office hours, after hours clinics, and a nurse-staffed Telephone Health Advisory Services (THAS).

Patient Enrollment Requirements

By completing and signing a Patient Enrollment Form obtained from your family physician, you as a patient commit to the following:

  • To seek treatment from you Family Physician or the FHT first, unless you are travelling or find yourself in an emergency situation.
  • To allow the Ministry to provide your doctor or the FHT with information about services you have received from other family doctors or walk-in clinics outside of the FHT, and some preventative care services.
  • To not switch physicians or FHT’s they are enrolled with more than twice per year.

A patient remains enrolled unless they are no longer eligible for OHIP, move outside of the geographic area of the FHT, enroll with another physician in another group, or choose to cancel their enrollment.

Please inform you physician if you are receiving specialized treatment from another doctor outside of Portage Medical FHT.
